The notice of appeal shall be in writing, signed by the appellant, and shall include:

A. The name, address and telephone number of the appellant;

B. A legal description and address of the property involved;

C. A brief description of the decision in question;

D. The reason or reasons given by the director of inspections for violation of this chapter and any part herein; and

E. The grounds on which the appeal is being made. (Ord. 1977-14 § 16-3, 1977)
